Federal Regulations Governing Facsimile Communications:
Beginning July 1, 2005, Federal law will prohibit the broadcast of unsolicited facsimile (“fax”) advertising to a facsimile machine without prior written permission of the recipient. An “unsolicited advertisement” is defined by Federal law as “any material advertising the commercial availability or quality of any property, goods, or services which is transmitted to any person without that person’s prior express invitation or permission.” A person may grant permission to send facsimile advertisements only with a signed written statement that includes a fax number to which advertisements may be sent.
In accordance with these Federal regulations Just Basic Sports, Inc. (dba Pizzazz), is requesting written permission to send fax communications, including but not limited to advertisements and notices, to the fax number included on your Requests and Comments form found at www.pizzazzwear.com. By submitting your Requests and Comments form you acknowledge that your typed name and fax number will serve as written permission authorizing Just Basic Sports, Inc. (dba Pizzazz) to send you fax communications. This authorization will remain in effect until Just Basic Sports, Inc. (dba Pizzazz) is notified in writing (either by mail or e-mail) that permission is no longer granted to send fax communications.

Federal Regulations Governing E-mail Communications:
Beginning July 1, 2004, the CAN-SPAM Act of 2003 will govern the broadcast of e-mail communications and solicitation on via the Internet. Federal law prohibits the broadcast of unsolicited commercial electronic mail. In order to receive commercial e-mail you must provide affirmative consent to receive such mail messages.
In accordance with these Federal regulations Just Basic Sports, Inc. (dba Pizzazz) is requesting written permission to send e-mail communications, including but not limited to advertisements and notices, to the e-mail address included on your Requests and Comments form found at www.pizzazzwear.com. Just Basic Sports, Inc. (dba Pizzazz) does not sell, share or in anyway disseminate e-mail address collected in course of normal business. Any e-mail address provided will be used solely by Just Basic Sports, Inc. (dba Pizzazz) to communicate with clients and potential clients regarding products, product updates, special programs, orders and other necessary business communications. By submitting your Requests and Comments form you acknowledge that your typed name and e-mail address will serve as written permission authorizing Just Basic Sports, Inc. (dba Pizzazz) to send you e-mail communications. This authorization will continue to serve as permission to send e-mail communications as governed by Federal law at the time. This authorization will remain in effect until Just Basic Sports, Inc. (dba Pizzazz) is notified in writing (either by mail or e-mail) that permission is no longer granted to send e-mail communications.
